CONTINUING CARE HOME TYPE B

(formerly known as SUPPORTIVE LIVING)

Intercare’s Kingsland Terrace is a 24 bed facility that offers an innovative model of ‘Supportive Living’. The facility’s programming focuses on residents who are under age 65 years of age and who have been diagnosed with a neurodegenerative disease such as, for example, Huntington’s Disease (HD), Multiple Sclerosis (MS), Downs Syndrome or Brain Injury.

The Supportive Living Team consists of:

  1. Site Administrator is responsible for the overall operations, administration and clinical care/services at Kingsland Terrace.
  2. Licensed Practical Nurses (LPNs) and Clinical Care Specialists (CCS) specialize in continuing care and are responsible for the provision of care and care-related services twenty-four (24) hours a day, seven (7) days per week.
  3. Recreation Therapist assesses Residents and, together with Activity Aides, provides Residents with choices and opportunities to participate in a variety of recreational activities.
  4. AHS Case Manager is accountable for working with residents and families in developing a plan of care that is individually designed for each resident and works together with the Intercare team to ensure that all Residents receive safe, quality care and services.
